max wattage on 120v ac USA ?

There is a tough to find data sheet that does spec this unit at a max of 30vdc. Refer to this link for more info, http://electronics.stackexchange.com/questions/97721/why-ac-points-on-pcb-are-separated-by-a-holeThere are products that are very similar and rated for 120v ac use.

Does it have optocoupler isolation?

No opto-coupled isolation. There is a small transistor onboard that is driving the relay and as someone else pointed out, the coil has a diode across it to supress the counter EMF of the coil. This relay requires very little to drive it as long as it's wired correctly ("+" to 5-12VDC, "-" to Ground, "s" to your digital output. The output is active high (when your output is high, the relay energizes). The screw terminals (NO = Normally Open, NC = Normally Closed, C = Common).

How reliable are these? I bought one but it just died on me, the led lit but it wouldn’t switch anymore so I had to throw it away.

I've had two of these in service for over 2 years now with no issues. They are each separately controlling garage door momentary switches. They are each in a circuit with an Arduino activated by bluetooth to start the garage door motor from the momentary switch. They've worked great for me.

Does this only work with official Arduinos?

No, it will work with anything that sends out a 5v digital signal (though I also had success with a 3v esp8266, so input tolerances are probably fairly wide )
