There are two pieces to this: the g-string pouch and the piece with the flaps. The pouch and flaps are made of 100% cotton, held together by elastic straps. The g-string pouch fits me well enough and is comfortable to wear - I plan to get a lot of fun out of it - but the flaps on the second piece tend to ride up, particularly in the back, so I may look at sewing a larger, longer flap piece under the factory flap to keep it down. I have a 36-38 inch waist (depending on whether its running season or not) so went with the XL per the sizing chart. Sizing seems correct. If you're an exotic dancer, you'll have some fun gigs with this. If you're just giving a "private performance", you'll have fun with it, too.
