It has about 20 tunes that it plays in the same order over and over.

Does the built in speaker always play the sound externally or can that be turned on and off?

The Built in speaker can be turned ON or OFF via a simple Slide switch, the device is always playing into the phone system independently.

Can i download opus number one to this?

You are able to create your own play list. So unless there is a file size limit that you would hit I can't see why not. My guess would be you just bump up to a larger thumb drive if that happens.

Will the mp3 track start from the very beginning as soon as the call is placed on hold? I would like for the person on hold to hear the entire message

No, it is continues loop and so are all other other music on hold players. There is another product called "Call Greeter Plus" that can take calls and place them on Hold (if you do not take the call) and then the mp3 track start from the very beginning in that case
