Are they made in U.S.A?

Not 100% sure but they hold up very well. I bought mine to remove drive pins on a backhoe bucket teeth. I beat on them hard. With the help of a torch and big hammer they did the job.

In order to warranty one of the punches do I have to go through Starrett or Amazon?

I warrantied one directly through Starrett as it had been some time since I first purchased the punches from Amazon.
