Do you have to open the bag of the bait before putting it on the floor?

The bait bag actually works intact or opened. I think the odor is stronger when the bag is opened and the pellets are stuck to the floor of the trap, but we tried it both ways and both ways work.

How long should I leave the trap out?

It's recommended to replace it every month because the bait loses it's attractiveness after a while. I had several set up longer but found they were less effectiveness after a month and the bait packet slid into the reachable zone on the traps I set on it's side. I suggest rotating the trap every week or two if set on it's side so the bait stays in it's proper location. This is still the best trap and defense against the roaches that I have found, especially against the ones I can't see. I have tried most everything. The number of roaches I have trapped has dropped and gotten smaller each purchase. The first time in years that I felt I'm finally getting a handle on the problem - I live in an apt where my neighbor the first 2 years, was infested and dirty. Need I say more?

Does this product work on the large german cockroaches?

Yes, this works on both American cockroaches (large (~1.5" long, winged adult & dark reddish brown), and German cockroaches (smaller ~0.75" long, winged adult & light brown/tan).
