How do you wear these? I see only 1 pouch. I just got these, and didnt think I'd need instruction, but I do not see 2 pouches.

There really is only one pouch. It is for your Dude and his buddies just sit as any other underwear. they shouldn't be advertised as "Two Pouch". I'm sure they are probably a great product with great Ball Sak support, but there is not two pouches. My fiancé and I were considering these, but thankfully her friends husband bought these and was slightly disappointed with only being one pouch.

Very confused on the sizing... Some say run large others too small. Yet I can't order a Small for a 33" waist???

I hate miss information. The medium, according to the box is 36-38in. These are not boxers, but trunks (I know, but I have to restate the obvious), they should be snug and ride low. If you are wearing normal pants, you should NOT see the elastic. The small size is 34-36in. They may be too big, and and I don't know if they come in XS.

Do these enhance your buldge?

While it's true these don't "enhance" your bulge, the pouch for your member doesn't serve to suppress what you have. In other words, most briefs take your boys and, thru sheer fabric force and tautness, compress your nether regions together until they conform to the confines and curvature of your garments. These, on the other hand, have a (as another reviewer said) hoodie that just pulls loosely over you tap root. I'm not the most endowed, but I probably won't be wearing these under thin gym shorts when I'm around the women-folk. The slightest bit of extra blood flow will surely be apparent.
