Can someone provide a complete list of the ingredients? I see the individual herbs listed and "liposorb" but I am wondering about alcohol etc.

In addition to herbs and liposorb, there are water, glycerin, ethanol, and vitamin E in Cocktail. (As you probably know, ethanol is an alcohol.) Again, sorry for listing the ingredients to a different liposomal product in my answer above.

It is not necessary to be kept cool but after opening you should refrigerate it.

I use this product but forgot its supposed to stay in the fridge and stayed opened for 6 days. Do i need to replace it?

Probably not...like many probiotics, it lasts better with refrigeration, but can withstand a few days left out. It won’t go bad...but may not have the power that refrigeration assures.
