My mom is complaining about the strap causing her skin to itch. Is there something she could put under it?

I have not had an issue with that, but I usually wear it over my leggings or jeans so it doesn't touch my skin. On the few occasions I have worn it with shorts, no problems, but if your skin is touched by the velcro then I can understand the discomfort. Put a piece of material between it and your skin, then cinch it up.

Would this be any help for sciataca?

This product would not help sciatica. Good for Osgood Schlatter.

Can you wear this over leggings or jeans, like skinny jeans and/or fitted pants?

Yes, I wear it over and under exercise pants. Either works.
