Q: What's the width and height of this diffuser?

28" wide19" tallKeep in mind the top "neck" folds down. (the black part in the picture) So if you have a small access hole you can fold the neck down allowing it to be inserted vertical. Then after its past you access point you can fold it back up so that the diffuser sits in the horizontal position. I have personally seen this diffuser bar be used in tanks that have access holes as small as 8"

Q: Would you use this in a single chamber tank with an effluent filter? What effluent filter would you recommend?

I wouldn’t even with the use of a “filter”. Since such a filter would be pretty much prone to blockage. Although you might try as long as you acknowledge this fact and you have the means to inspect and stay proactive. Then again, I wouldn’t. Hoping the best for your project, blessings from Atlanta!!

Q: How do I connect this to my weighted tubing?

I drill a 3/4” hole into the side of the tank and used an 90 elbow to connect it to the diffuser.

Q: How does black neck fold down ? is it flexible?

Black neck has two slip elbows that allow 360 rotation.
