Q: What is the real magnification of the 5x/500% model? Is it 5 diopter (i.e. 2.25 time magnification) or an object will really be 5 times larger?

This product definitely does NOT magnify by 5x. I tested thoroughly, and can't get more than 1.75x with an object in focus. At the furthest in-focus distance, 1 inch measures at 1.75 inches (only possible with eyes and object equidistant at 1.5 feet).

Q: Will this work at 220v or 110v only?

It is a 5VDC USB powered. If you have a 5v USB power supply min 1 Amp for 220V it will work. [I haven't kept any power supply with its "assigned" device in years] at lowest brightness it draws about 1A (5 watts @ 5VDC), But draws 2.2 Amps (11 watts @ 5VDC) at max brightness so most any USB plug-in will work, frequently with 110VAC OR 220VAC. Wonderful little thing, I don't use my 6" Fluorescent Ring 12 watt @ 120VAC much anymore (it uses about 25x more power for a slightly larger glass. Only complaint is Ny-Lock nuts get loose, and then it droops until I tighten w/ my 7/32", 6mm, needlenose or 4" crescent wrench

Q: After 10 months, lamp assembly keeps slipping off table. I try to keep clamp tight, but without success. Any suggestions?

Try laying a hand towel down so that both sides of clamp have a grip before you tighten down.
