I bought these to replace one of our bags that broke. They're so handy to have around the house. I use the large one for my shoulder injury but the smaller ones are great for headaches, joint pain etcI tried using the strap and it wasn't great for me, but may work others. I prefer to just position the bags and then support myself with cushions if I need toGreat product
