is cutting surface 4.5"?

No the scissors from top to bottom are 4-1/2”. The blade/cutting surface is only 1-1/4”.

Is it multi-autoclavable surgical scissor or not , please??

While I have not used them for that, they are Dental Surgery scissors, used in health care settings, so, given that and how sturdy they are, I would say yes. These are incredible scissors, I wish all my scissors performed the way these do, true luxury to work with.
