How reliable is the test?

My advice would be to see your doctor and have him send you home with the lab kit-- which you rather easily complete and bring back to the lab. Sometimes you must pick up the kit at the lab and return it to them. Having an easy to follow test is better than doubting whether this product on sale is going to give you peace of mind.

Is this test to be used once a year?

It is so inexpensive and easy that I use it several times a year.

how does fecal stool test relate to basal skin carcinoma

There is no relation at all to basal cell skin tumors. Fecal stool for blood tests for colon tumors only.
