Yes, this pillow does have a cooling effect. However, if you are using a heavy or flannel pillowcase, the cooling is not as strong. I also find that when it colder outside, the gel cooling pad is cooler.

Q: Does this come rolled up or flat in a box?

It come in a rolled bag . It will be at flat shape in 2 hours .
