It sounds like I can use it to speed up the heating of a small in ground spa?

THE 1000 WATT heater element (Outer Box Labeled 1000 Watt, Label on actual product states 2500 Watt). The Measured operating current by me on the Heater probe I received from Amazon was 8 Amps ... at 125 Vac = 1000 Watt. It takes approximately 20 minutes to heat a 5 Gallon water bucket to a temperature that is slightly hot to the touch. Typical Hot Tub = 300 to 450 gals Gallons ..... divided by 5 Gallons x 20 minutes = 20 hours to 30 hours. Cold Water starting temperature was approximately 60 F.

Can this be used in a hot tub while youre in thd hot tub to maintain heat?

not recommend to do this.even it should be safe when you are in the tub.

Can this be use in a pool to warm up?

If you mean a kiddie pool, yes.

Does it have to be just plain water?I want to use it in an Epsom salt bath
