Is the app and wifi required for the scale to work?

No it connect via bluetooth. I recommend this scale due to its quality and the app. It truly is worth the money!

how accurate is the body fat percentage etc?

Body fat I think it very accurate as I was always told once u can see abs in the Mirror u r at 15%. I gave in the past done the water test and the pods so I think this is a good reasonable measure

Can it only use its APP? I'm used to Apple Health

You can sync data to Apple Health

How good is this scale without the app and is it still worth the money?

Even without the app it will read your weight, body fat percentage, BMI, muscle mass, body water percentage and bone mass. If this is what you want then there is no need to download the app. The app is worth it!
