Are the box and/or the bottle sealed with a tamper-resistant seal?

When you purchase genuine product from TheFitterCritter, the box is not sealed, the bottle has a "click-lock" style lid, there is not an additional seal under that safety cap, and the dropper is sealed in plastic.

Does this product come with a dropper in bottle?

When you purchase authentic product from our company it comes as shown, a 1oz bottle in a small cardboard box with a dropper inside the box.

If its 1/2 ML per 5lbs for a puppy, how much do you give a toy breed thats 1.5lbs?

Dosage works out to 0.1 ml/lb. So your puppy would get 0.15ml right now. I'd round up to 0.2ml. Adjust the dosage as he/she grows. The little extra given in rounding up is fine.
