Does this device have to use the timer? Can it stay on and off without using the timer? Is the the timer just an additional feature?

You do not need to use a timer with this device. Additionally, it has three heat settings: low, medium, and high.

What's the wattage for each setting hi/med/lo ?

Not sure wattage. I used in sub freezing (20*F) weather on lowest setting and had no fogging/dew on Nikkor Z20mm 1.8, Nikkor Z70-200 2.8, and a Tokina 14-28 2.8. Works well, surprisingly low cost. (I use a cheaply rechargeable battery bank)

Does this unit automatically turn off? i need a lens warmer that will last 4-5 days without turning off.

Only if your battery runs out. No thermostat… just constant heat settings
