Yes, the Multi Sim package includes cut outs for standard, micro and nano sized sim cards. Most recent iphones use nano sim cards.

Can I activate the phone & plan online, with a U.S. credit card & address?

We learned that we were not able to activate the phone or buy a plan in a Canadian Bell store. We were able to activate the phone (the sales people in the store told us to find Internet access somewhere like Walmart!) We were never able to buy a plan online because the form would not accept a US address for our credit card. Spent our 1 1/2 months in Canada without a phone.

Yes, once activated, this SIM will have a Canadian phone number attached to it. It can be topped up online with any credit card so long as the primary mailing address attached to the card is a Canadian address that matches the address on your Bell account. Otherwise, you'll have to buy top-up cards. Note that Bell's prepaid plans don't provide the option for roaming in other countries (e.g. the US) but do include roaming across Canada.

I registered for use and it really didn't save me any money, and then found out our cell phone worked in Canada without having to change sim cards.
