i dont know about silent..Well yes its silent because its rubbish, bought this for my wife, it lasted less than 2 weeks, the paint came off, something inside broke and it rattled. Poorly made. id advise getting a different make, like a Tecknet or something else they are just as cheap but much better made, wife had had her Technet wireless mouse now over 4 months and had 0 problems

Does the scroll wheel work as a middle button i.e. does it work as 3 button mouse?

It does - But beware: I bought this and it lasted about 30 mins before something broke. If I remember correctly it was the wheel mechanism that dissolved.
