For the wire connections on these, is there a clamp plate or just the screw to provide the connection?

This is a commercial-grade receptacle. You can install your wiring on the screws with a shepherd’s hook or you can wire straight into the back of the receptacle with a straight conductor. If you use the back-wire method, the screws and clamp plates will secure the wires. Each screw has two holes on the back of the receptacle if daisy-chaining is necessary. This outlet does NOT have the backstab connectors.

Is 12/3 Romex required?

12/2 romex with ground would be sufficient since the device will not be requiring a neutral. The run for this device will need to be on a double 20 amp breaker in the panel. The white conductor in the panel needs to be marked with black tape to indicate it is a hot conductor and connected to one pole of the breaker with the black conductor connected to the other pole of the breaker. The outlet will service 220 +/- volt loads.
