Tried this recently and the bag barely got cold. Has anyone else had this issue?

I've just received this and ot works an absolute treat. I broke my ankle 2 days ago and the 28cm bag with some ice and water on wraps around nicely and really feels like it's easing the pain, swelling seems to be reducing too

Are the dimensions i.e 12cm, 20cm & 28cm from top to bottom as comes packaged or is that the circumference when opened up and empty/filled?

Can we use it with warm water as well?
