What is the actual diameter of this apple? I've read both 2 inches, and 3 inches.

Think 2 and a half inches -- it depends on where you measure the circumference. If you are concerned about overall size, think of it this way -- the bottom that rests on a table, desk or shelf is about the size of a small bottle of eye drops. The height (without the stem) is about the size of the height of the eye drops bottle without the squeeze-top or lid. It's heavy enough that it is not easily knocked over, which is important if you use it as a paperweight. The bright red color and quality of the crystal make it stand out even across a room.

Is the red color only on the surface of the glass, or is it red glass?

I have the red and green apple. The color is within the glass, not just on the exterior. These things are great.

Can this item be inscribed?

This one does not come inscribed.. i don't see why you couldn't take it somewhere, there is another one here on amazon that I bought that was clear and they inscribed whatever you wanted on it..
