Q: Can you fill it from the top?

No, only the bottom opens. You have to take it off of the kennel to refill it

Q: How does this attach to the dog crate? There should be instructions included.

I hooked it to my kennel door wide.. my dog doesn't make mess with water spilled in kennel anymore.

Q: I have a German shepherd , is this good for large dogs?

It is my belief, this bottle would be too small for a German Shepherd. Although it is perfect for my Shih Tsu, the bottle has to be refilled may times during the day. Hope this answer is helpful .
