Is it easy to assemble?

It's not that complicated. You just have to stack the wires together one tier at a time. It took a few minutes to figure out where they go but there aren't too many combinations to try out. I recommend to keep the plates away from you while you are assembling it.

I did not keep the box so do not know. It is not stamped on the plates.
