Q: would this work to repair a torn sheet/duvet cover (4 inch tear)

This product is non-woven, so it would not add any strength to the torn fabric. I would recommend something like a two-sided fusible stabilizer and fuse it between the sheet/duvet cover and a strong woven fabric, using a piece much larger than the tear so it will have some strength to it. If the sheet is very worn, though, it will only tear in other areas. It might warrant buying a new sheet/duvet cover. Hope this helps.

I haven't tried it, but I think it will be fine
