Can the screen be used for anything other than the personal trainer?

The monitor is limited. It is a touchscreen. And it connects directly with the rower. You can output via HDMI to a second screen. But you can not input (like from an Apple TV or Blu-Ray player). The monitor starts up and loads directly into iFit. This happens whether you have a subscription or not. The monitor still has some features without a subscription. To try to do anything to it requires you to unscrew the whole thing and void the warranty. I got an Ipad holder and connected it to the rower. I row in manual mode (no live trainer) and watch videos on the Ipad.

Can you use headphones with this?

Yes. There is a headphone jack for wired earbuds. No Bluetooth support. I did buy a thirty dollar Bluetooth transmitter to plug into headphone jack so that I can use with Bluetooth wireless earbuds.

Where is this rower made?

I actually tried to Google that. It appears that a lot of it is made in China. I just got done shipping it back. others have had luck with it but there’s also been numerous others with issues. I settled on the concept2 with no TV. I’ts been around a while and solid as a rock. I had nothing but a headache with Nordic and icon customer service was less than helpful. They were more than happy to send me any parts that I might need but expected me to be the repair technician. I would just make sure you read all the reviews you can find before making a decision. Video demonstrates the issue I was having right out of the box. Others have had the same issue.
