Q: where can I buy the spray itself from this same brand?

Q: Does this move around on the toilet while using or is it secure? Seems like its flat & will move.

It is not flat. There is a part that is about an inch under what you see in the picture. That is what holds it in the bowl of the toilet.

Q: How big is the opening at the bottom? Is it just a slot or is it the entire bottom open?

The entire bottom is open, a CD could easily fall through into the toilet (this has happened to me a couple times where it will slide down from clip while spraying and into toilet). I'm not sure of exact measurements of the opening.
