Q: Is this the one that smells like Fierce cologne?

Smells good but i dont recommend it for car usage.

Q: So this may seem idiotic, but how do you even get these things to work? I tore off the top like it said, but there is no smell coming from it..

It most likely works fine. My guess is it is perhaps activated by some heat and air flow. I did not think it was very strong either but having placed in the holder and attaching to my visor has worked well. Subtle but well. My guess is it may be too subtle for your application.
