The drain cover has a slight rise above floor level but it is quite comfortable with no sharp edges. Unless there is a specific reason you need the cover to fit perfectly flat, it's not a real issue.

What is the distance between prongs (opposite sides) and height of the prongs?

3 inches apart, and appx 1.25 inches long. The slight lip around the cover shortens the inserted length just a bit.
