What happens if the dog has diarrhea on the grass?

You can wash the grass, I have actually tossed it in the washer and the dryer to get it a bit cleaner then just hosing it off. Some of the grass does come loose by doing it this way but its easy to clean it out of the washer or dryer. I have a chihuahua and live in an upstairs condo, so this grass is perfect for her.

It depends. If your dogs can hold it for a long time, they may not even use it. You may have to train them to use it. Once they do, and they smell their scent, then they might use it.

How can I clean the grass mat?

I take it outside and use soap and hose it down.
