can it be used to extract insect venom?

It works well to extract insect venom if done before the injection site closes. I bought this for a medical mission in the Amazon and it helps to at least lesson the irritation.

do you just pullup the plunger to create suction?

No. You pull up the plunger all the way, then position it firmly on the skin just where you want the suction. Then depress the plunger ALL THE WAY DOWN. You’ll instantly feel your skin rising up inside the transparent attachment. When I’m trying to suction an insect bite on my arm, and I can’t get my hand positioned to push the plunger down, I use my chin to push the plunger all the way down. I love this gadget!

Referring back to my original review. This is a P.O.S. product. Poorly made, cup is way too small, wasted my money buying it, don't waste yours. I live in snake country (west Texas) and I'm not trusting my life to it! To answer your question though, it would be reusable after a thorough cleaning.

Probably not. Do you know what the odds of being bitten by a snake in the US? Unless you're somewhere in the "DEEP" jungle the odds are practically nil. You might try Googling "Ever Ready First Aid Snake Bite Venom Extractor Kit testimonials". That being said, the device does have other useful First Aid applications. I would also suggest reading up on emergency 1st aid treatment for snakebite. Things have changed quite a bit within the past decade or so.
