Can the dog move around in this carrier or lie down?

My dog is 4 lbs. and she can turn around and lay down, with room to spare.

Can this be worn over just one shoulder, on my side? and not across my front?

I carried it on one shoulder without the dog in it but it is constructed to be more comfortable for you and the dog to be worn in front.

What size is best for 3.4 lb puppy who will grow to 7lbs? Small or Large?

NO. It was way too big and bulky for my 7 pound chihuahua. My daughter got me a perfect over-shoulder "purse" type carrier that works perfectly. Don't know where she got it.
