Are there actual holes in the center?

Unfortunately yes, It is my only complaint about them. The condensation from a cold glass will go through and on to the surface.

I use a minifridge as an end table, and sometimes the top can get warm and melt my cold drinks, do they insulate against heat at all? Are they thick?

They are as thick as a regular record is. It may insult a little. Or you could make a round piece of cardboard to help

Do they fit in the cup holders of a car?

I have a Ford Edge and they are def too big for my cup holders.

Can they handle heat from a mug? One that has coffee or tea.

Yes, they can handle the heat and moisture! They have a no slide ring on bottom too! We love ours! They are fun!
