does this knee exerciser have tension adjustments? Looks like it takes fair tension amount of arm/shoulder strength

No tension adjustments I can see or aware of. This is simply to slide your foot closer to your butt. You move it with your knee first and then use the pulls to bring your foot back as far to your butt as you can. It is stretching the knee.
