Q: will it fit 4 inch thick full foam

I have one on each of two 8" mattresses and one on a 12" mattress. Seems to conform to both the same. Also it is comfortable to sleep on. I have it on my own bed and never even know it's there. I would go ahead and try it on a 4" as it seems to tuck in well. It's a good product for the price. You can always send it back easily for free with Amazon if it doesn't work out.

Q: Will this fit securely on a 6" thick futon mattress? It's a standard full 54x75.

I have mine on normal twin mattresses (not extra long). They fit great with room for a thicker mattress but enough elastic to hold it so it's not loose and bumpy. I would say if you can use standard fitted sheets, this will fit.

Q: what is the country of orgin

The cardboard inside the plastic zip cover has nothing about country of origin listed. It may be on the mattress protector itself but, at this time, I cannot find out, because it is being used in the guest bedroom. Sorry.

Q: Will this fit a hospital bed mattress?

I use this mattress on a memory foam queen size sofa bed and it fits well. Queen size sheets are way too big for this mattress but the cover works well on this mattress.
