Why don’t they have it with black skin as well

Unfortunately LEGO did not make this figure with a black face.

How does this baby attach to lego minifigures?

Baby Ola is VERY tiny (part of it's charm), but does not have an attachment feature. Easily lays in the Lego stroller sets which comes with its own baby. Check out 60134, 60204 or 60134. Also check the set with crib, stroller, bottle and baby as shown on Amazon only without a set number.

Can you switch the face? Does it open in any way?

HI, No you can't. It comes as one piece that you can attach to either any LEGO figure or lego piece, such as a crib or stroller etc. :) - Sofia
