No, it only holds the 8 1/2 X 11 size.

Will it hold a spiral notebook on the right side?

No it will not! I tried a standard spiral calendar and can not zip the portfolio! Left side will not hold standard folders either 😩 can not zip it still!

Can you fold the left side back to use like a clip-board when standing or will it ruin the spine of the portfolio?

Good question, but I think it is hard to fold the left side back to use like a clip-board because there is cardboard inside the body material. What's more, this style is complicated, it is not good to fold when using it.
