Does it come with a case

No it comes in a cardboard box. This is for a deskphone only not USB. If you a USB headset search for C5220

Do i need a connector lead to use this on a computer usb and is so, which one? I have voip telephone system on Mac and need a good headset.

Plantronics do make a QD to USB adapter to quote the manufactuer 'Plantronics provide two different adapters for analogue headsets. These are the DA70 and DA80 with inline control functions as volume and mute, and answer if soft phone client supports it.'. We hope this assists you.

Does this headset plug directly into a standard desk phone, or will I need to purchase an additional adaptor?

No it doesn't, business headsets usually require a separate bottom cable to connect to phone - this is intented to be helpful as it enables the user to swap the bottom-cable and retain the headset if systems are changed or use the headset on a variety of different base units with differing output sockets, simply by buying a replacement cable.
