Is this the headboard and frame together or do you need to buy both separately?

Together, I was surprised by the quality of the product, it is great!

If I use my box spring with this bed how much of the headboard will show? I like a higher bed.

I guess it depends on how high/thick your mattress is but I’d say you would have about 1 foot of headboard left with a boxspring.

What to do if my mattress requires full surface foundation?

I'm a big heavy guy, so just to be extra cautious, I put some thin plywood (3/8") across the slats under the mattress. If you get it cut just right, you won't even be able to see it. but I think it's only needed if one of the users/sleepers weighs 250 pounds or more. Even then I'm not sure it's really needed.
