Q: Is this to be attached to the door of the cabinet? My cabinet has a door that swings open. Is there a handle that you pull on to slide it out?

It attaches from the bottom. No handle included but you can sure add one. You could attach your door to the cabinet. Thank you.

Q: Can this be mounted to floor in a cubby hole & still glide outward? Or does it need to be off the floor?

As long as there is nothing obstructing it from extending, there's no reason it couldn't be. The metal rail on the bottom needs to be screwed into a bottom, and the rail on the side needs to be screwed into a side.

Q: Are the shelves adjustable?

The bottom shelve is adjustable. Thank you.

Q: To use in a cubby space would I need any otherparts for it glide out properly?

You should not, it is made to be on a flat surface. Thank you.
