what size s the set screws? thank you

The ones I received are 3/32". That's way way smaller than 4mm, 2.38 mm to be exact. I did not have that size, but do now.

3/8" fit for 9mm pipe?

3/8" equals 9.52 mm so if your pipe is 9 mm the collar will slide on . There might be to much clearance for what you are specifically doing - just depends. If your usage is extremely critical then pursue a mm dimensional collar, if not a 3/8" will suffice.

To hold a 5" wheel 5/16 bore, using 5/16 collars, should I also use 5/16 flat and lock washers ?

I would say a flat washer. But be sure to dimple -drill a shallow hole where the set screw will engage the shaft. Tighten the setscrew down add lock tight also.
