What is the precise length of these refills?

It's been 2+ years since I purchased these. I no longer have them. Sorry

How/Where do you take it apart to insert the refill?

On my pen you unscrew the base and you see the three pen refills. You have to pull them out and then press the new refill into the top of the pen. Be sure to do one at a time so the colors match the pen selectors on the top.

So, blue and red state 1-pk pouch but black say 12-pk at the same price. So how many are for real?

red, blue, black you get 1 each for just the blacks you get 12. All depends on if you use the other colors. I bought the 3 color pack and they worked great.
