Doesn’t it need a spray head at the opposite end of the stretchy end?

It has a spout at the end not a spray head. For bathing a pet and cleaning a shower there is no need for a spray head.

Can the outflow be adjusted?

You can turn your faucet on lower pressure. Too much pressure will blow it off.
