What is the insert card for?

The "insert card" slot is for a card, either license or credit card, to make a cell phone stand. Put the card in, leave enough for a shelf in front, the rest of card is in back as a base.

Does it come with explanation of useages?

I am not positive as it was a gift, however I believer the answer is no but use is like different screwdrivers etc.

what are the two holes for just below the ruler?

The 2 holes near the ruler is to attach a Wallet Ninja Money clip.
