After starter is started when do you start refrigerating it?

I'm definitely not an expert, but I believe you only need to refrigerate it if you're not going to use it for a while. If you're going to bake bread every few days or once a week, you can leave out the starter. That will help it grow. If you're not going to use it for at least 2 weeks, you can hold it in the fridge.

Is it normal for the jar and lid to have pits and imperfections in the glass?

My jar and lid do not. It could have happened in the firing process. If you are unsure, send back for replacement.

Can they go in the dishwasher?

Yes I put mine in the dishwasher. Does yours have a rubber gasket? If it does I would had wash and dry it by hand, Mine does not.
