Hello, it can be used on a mattress with box spring. Please measure the height of the mattress. Suitable mattress heights range from 2 to 8 inches.

Q: How many rails fro bottom to up ?

It looks just like the picture. 1 rail at the bottom and the other at the top. Very secure and it won't slide out. I screwed to 3ft sections of wood 1x2's to the bottom of each rod so it would keep it from sliding out. My Husband was 6'3" and even he couldn't push it out from underneath the mattress. I would say it will not break from a weight pull, however, unless you firmly attach the rail to the underside of the bed, it will work itself out and could fall free from the bed. The end result could very well be an unexpected tumble from the bed as the rail comes free with you holding on, or rolling into it while asleep.
