OK, if these are truly "personalized" glasses, why are there no instructions for doing that?

They aren't actually "personalized". They make the same glass with different years. I suggest you do a search for the year you need and if they make that year, which most likely they do, it will pop up.

is this dishwasher safe?

I purchased it for a friend and he only hand washes it, so I am uncertain.
