How do you refill the marker?

This is just a refill. But if you have the fat pilot marker, the top of the marker twists off.

Can I fill a fountain pen with this?

A bad idea. This is solvent-based. Fountain pen ink is water based. Fountain pen ink is dirt cheap. A small bottle lasts for a long time. Just buy fountain pen ink not this. (And ink jet ink? NO. Another really bad idea, for the same reasons.)

can this be used in brush pens?

Only a person who has your brush pens would know. But you do, so is the ink in your brush marker just like permanent markers? The other answer (about clogging) makes no sense. Permanent marker ink is a dye, not a pigment. So it's a fluid, which won't clog anything. (A pigment is tiny particles, so it might clog something, but permanent marked ink contains no pigment, so no clogging.)
