Q: Does it have a water filter or should water be filtered already?

Q: Is the water bottle included iif sowhat size?

No it is not included. I purchased 2 bottles separate from the water cooler in 3 gallon size

Q: It has 3 indicator lights on the front. What is the middle one & how does it work? There are only 2 water levers

The indicator light on the left is "Power", the middle one is "Hot", and the one on the right is "Cold". The switches to control them are on the left side of the unit.
